2019年，荷兰奈梅亨大学医学中心的研究者在Journalofcriticalcare杂志发表的一篇文献，探讨了流感与ICU住院和死亡率相关的特定因素，文章中也讨论了糖尿病合并流感患者的风险。阿宾顿-杰弗森健康中心的NeilSkolnik博士对这篇文献进行了播讲。接下来，我们共同聆听专家的解读。

Therearecertaingroupswhoaregreaterrisknotjustofgettingflu,butalsoofhavingseverecomplicationsfromtheflu.Letmejustgooverthreerepresentativestudiesofwhichtherearemanymore.AndthefirstisfromtheJournalofcriticalcare,lastyear,2019.AnditstitlewasInfluenzavirusandfactorsthatareassociatedwithICUadmissionandICUmortality.

某些特定人群不仅感染流感的风险更大，而且还会出现严重的流感并发症。这样的研究有很多，下面我来回顾三项有代表性的研究。第一项研究来自于去年，即2019年的《危重护理杂志》，其标题为《流感病毒以及与ICU入院/死亡率相关的因素》。

Now,thereasonfordoingthestudyisthatwhilemostinfluencethepatientshaveself-limitedrespiratoryillness,about5%to10%ofpatientshospitalizedwiththefludevelopedsevereenoughdiseasetorequireanICUadmission.TheaimofthestudywastoidentifyspecificfactorsthatwereassociatedwithICUadmissionsaswellaswithmortality.Theylookedatalmost200influenzapatientsadmittedtotwoacademichospitalsintheNetherlandsbetweenJanuary2015andJanuary2016.Andtheyinvestigatedwhichofthe45patientsoutofthe199wereadmittedtotheICU.

进行这项研究的原因是，尽管大多数患者罹患自限性呼吸系统疾病，但约有5%~10%的流感住院患者病情严重到需要进入ICU。此项研究旨在确定与ICU住院和死亡率相关的特定因素。研究者们观察了荷兰两家学院型医院2015年1月～2016年1月收治的199例流感患者。其中，共45例进入ICU。研究者对这45例患者进行了调查。

Whattheyfoundwasthatahistoryofobstructivesleepapnea,ahistoryofMI,dyspnea,influenzatypeA,BMI>30,thedevelopmentofrenalfailureandbacterialorfungalco-infections,wereobservedmorefrequentlyinpatientswhowereadmittedtotheICU,comparedtopatientsruntheregularhospitalflows.

他们发现，进入ICU的患者，既往罹患阻塞性睡眠呼吸暂停、心肌梗塞、呼吸困难、甲型流感、体重指数>30kg/㎡、肾功能持续恶化，以及合并细菌或真菌感染等病史的频率，高于常规住院患者。

Co-infectionswereevidentinabout55%ICU-admittedpatients,comparedwith20%ofpatientsonthehospitalflows.But,andgetreadyforthis,becausethisisveryimportant,non-survivorssufferedfromdiabetesandrenalfailuremoreoften,butnotjustalittlebitmoreoften.IftheylookatICUmortalityfrominfluenceofthepatientsadmittedtotheICUs,aboutathirdpassedawayfrominfluenza.NotofthoseadmittedwithinfluenzatotheICU，athirdpassedaway.

约55%的ICU患者存在明显的合并感染，在常规住院患者中，该比例为20%。但是，（接下来要引起注意，因为这很重要），特别容易（而非轻微）罹患糖尿病和肾功能衰竭的人，并没有存活下来。如果观察ICU收治病人的死亡率情况，会发现大约1/3的人死于流感。在ICU收治的非流感患者中，死亡率为1/3。

Butpersonswithdiabeteshadanoddsratioof7.1fornon-survivalfrominfluenzaiftheywereadmittedtotheICU,chronickidneydiseaseinoddsratioat6.8.Sowe'retalkingabouttheincreasedriskthatcomeswhenyouhavethecombinationofdiabetesandinfluenza.Notasmallamount,butalargeamount.

若合并流感的糖尿病患者被收治到ICU，其存活优势比为7.1；合并流感的慢性肾病患者若被收治到ICU，其存活优势比为6.8。所以当患者同时罹患糖尿病和流感，其风险会增加。这个风险很大，并非说说而已。

当我这么说的时候，不禁联想到我们目前正在应对着另一种疾病的广泛传播。糖尿病的病因，我们目前还未完全了解。对于合并感染的患者，糖尿病已经是造成其不良临床结局最主要的或几乎最主要的风险因素之一。
